INFORMATION / GUIDELINES
- This application must be submitted at the latest within TWO DAYS from the day the benefit is due.
- The application must be accompanied by the following:
(a) A medical certificate stating the nature and duration of incapacity for work.
(b) A certificate of attendance for each single dependent child of the applicant aged between 15 and 23 years if she is a daughter or between 15 and 25 years of age if she is a son.
(c) Certificate of service for each single son of up to 25 years of age serving his term in the National Guard.
(d) Medical certificate for a disabled single child over 15 who is permanently incapable of work and is being maintained by the applicant.
(e) Birth certificate for each dependent child if the application is submitted for the first time.
- The application must be filed within twenty-one days from the date that the benefit is claimed.
- Together with the application, the following should be submitted.
(a) A medical certificate specifying the nature and duration of the work inability.
(b) A school attendance certificate for each dependent single child. For girls aged between 15-23 years old. For boys, aged between 15-25 years old.
(c) A military service certified for each dependent child, up to 25 years old, doing his service in the National Guard.
(d) A medical certificate for a disable, single child over 15 years old who is permanently incapable of working and is supported by the applicant.
(e) A Birth certificate for each dependent child if the application is submitted for the first time.
